This policy explains what information Music City Ketamine ("we," "us") collects through our website, our online booking form, and our advertising on platforms such as Facebook, Instagram, and Google — and what we do with it.
This is not our HIPAA notice. Information you submit through this website or an ad before you become a patient is contact information, not protected health information. Once you establish care with us, your medical records are governed separately by HIPAA and by our Notice of Privacy Practices, which we provide to you at your first appointment. Information we collect
- Information you give us. When you request a consultation — through our booking form, a contact form, a lead form on Facebook or Instagram, a phone call, or a text — we collect your name, email address, phone number, and anything else you choose to tell us.
- Information collected automatically. When you visit the site we and our analytics and advertising providers may collect your IP address, browser and device type, pages viewed, referring page, and general location (typically city-level), using cookies and similar technologies.
We do not ask you to submit medical or health details through this website, and we ask that you not include them in web forms. Please save those for your consultation, where they are handled under HIPAA.
How we use it
- To respond to you, schedule a consultation, and follow up about your inquiry — by phone, text, or email.
- To operate, secure, and improve the website.
- To measure and improve our advertising, including understanding which ads lead to consultation requests.
- To meet legal, regulatory, and recordkeeping obligations.
Service providers we use
We share information only as needed with vendors who help us run the practice, and we do not sell your personal information. These include:
- HighLevel (LeadConnector) — our booking form and customer messaging system, which stores the contact details you submit.
- Meta Platforms — the Meta Pixel and Facebook/Instagram lead forms, for advertising measurement and delivery.
- Google — Google Analytics 4 and Google Ads, for site analytics and advertising measurement.
- Cloudflare — website hosting and delivery.
These providers process information under their own privacy policies. We may also disclose information if required by law, or to protect the rights, safety, or property of our patients, staff, or practice.
Cookies and advertising choices
Our site uses cookies and pixels for analytics and advertising measurement. You can control cookies in your browser settings. You can also limit ad personalization directly:

You can unsubscribe from marketing email using the link in any message, or by contacting us. Opting out of marketing does not stop messages directly related to an appointment you have scheduled.
How long we keep information
We keep inquiry and contact information for as long as needed to respond to you and for our legitimate business and legal purposes, then delete or de-identify it. Medical records, once you become a patient, are retained under Tennessee law and our HIPAA policies.
Your choices
You may ask us to access, correct, or delete the contact information we hold about you, or to stop contacting you. Email or call us using the details below and we will respond as required by applicable law. Depending on where you live, you may have additional rights under state privacy law; tell us and we will honor them where they apply.
Children
This website is not directed to children under 13, and we do not knowingly collect their information. If you believe a child has given us information, contact us and we will delete it.
Security
We use reasonable administrative and technical safeguards to protect the information we hold. No website or transmission over the internet is completely secure, so we cannot guarantee absolute security.
Changes
If we update this policy we will revise the effective date above and post the new version here.
